Instructions for Completion of Baptismal Record Form San Francisco Solano Catholic Church

Please fill out the following Baptismal Record Forms below these instructions completely and accurately. Since the information you provide will be transferred to our permanent baptismal records, it is important that you provide complete and correct information. Please print legibly. Please pay special attention to the following:

  1. Child’s Full Name (First, Middle, Last)

  2. Place of Birth (City, State)

  3. Father’s Full Name (First, Middle, Last)

  4. Mother’s Full Name (First, Middle, Maiden Name)

  5. Godparents’ Full Names (First, Middle, Last) and Religion—Roman Catholic

Do not use abbreviations, nicknames, or short versions of names.

If you are married in the Catholic Church, you may proceed with the scheduling of the baptismal class and date of your child’s baptism. However, if you are not married in the Catholic Church, no baptism may be scheduled nor may you attend the baptismal class until you first meet with either the Deacon or Parochial Vicar assigned to San Francisco Solano Parish so that you may discuss with him the possibility of your celebrating a marriage in the Church, enabling you to participate fully in the sacramental life of the Church. Please call the Parish Receptionist who will arrange for you to call either the Deacon or Parochial Vicar to schedule an appointment. (Please see Addendum, “Marriage in the Roman Catholic Church.”) Once you have met with the Deacon or Parochial Vicar, you may arrange with the parish office to attend the baptismal class and schedule the date of your child’s baptism. We do not use this form to schedule a baptism. There is a limit to the number of baptisms scheduled on the designated Sundays. If a date is full, you will be asked to schedule the baptism for another day.

Sponsors (Godparents) for the Sacrament of Baptism

One Sponsor (Godparent), male or female (not dependent upon whether the child to be baptized is a boy or girl), is sufficient for the Sacrament of Baptism, but there may be two, one male and one female. If there are two Godparents, there must be a Godmother and a Godfather, not two Godmothers or two Godfathers.

A Sponsor (Godparent) must be a Roman Catholic at least sixteen years of age who has been confirmed and has received the Eucharist and who practices his or her Catholic faith, which befits the role of Sponsor (Godparent). If married, the Sponsor (Godparent) must be married in the Roman Catholic Church.

A baptized non-Catholic may stand with a Catholic Sponsor as a Christian Witness to the Baptism, but not as a Sponsor (Godparent).

A non-baptized person—a person of a non-Christian faith—may not stand as a Sponsor (Godparent) or Witness.
It is customary for the family of the newly baptized to make a donation to the church on the occasion of baptism. An offering by check may be made payable to San Francisco Solano Church. An envelope will be provided.
