Eucharist Ministry to the Homebound

If you have a medical reason for not being able to return to church and would like an Extraordinary Minister of Holy Communion to come to your house and bring you Holy Communion, please fill out this form so we can contact you to set up a delivery time. It may not be on a Sunday but during the week, depending on the availability of our EMHC.
